How to calculate therapy billing units from minutes

Formula and example

A timed CPT code can be billed for one unit once at least 8 minutes of that service have been provided. Each additional unit generally requires another full 15-minute increment, with the exact cutoff points defined in the chart below rather than a simple division.

  1. 15-minute round-down formula: Units = floor(Total timed minutes ÷ 15
  2. 15-minute midpoint formula:Units = round(Total timed minutes ÷ 15)

Worked example:

  • Total timed minutes: 53
  • Medicare 8-minute rule: 53 minutes falls in the 53–67 minute range → 4 units
  • 15-minute round-down method: 53 ÷ 15 = 3.53 → floor → 3 units
  • 15-minute midpoint method: 53 ÷ 15 = 3.53 → rounded → 4 units
